    Does PENG-LFCN Block Have Significant Optimization Effect?

    Objective: To compare the clinical effects of pericapsular hip nerve block (PENG) combined with lateral femoral cutaneous nerve block (LFCN) and hourglass iliac fascial space block (FICB) in total hip replacement (THR) in elderly patients. Method: Sixty elderly ASAⅡorⅢ patients of both sexes, aged 65-85 yrs., with BMI 18-25 kg/m2, were divided into two groups by random number table method: FICB (group F, n=30) and PENG-LFCN (group PL, n=30). FICB with hourglass method was performed under ultrasound guidance in group F, and PENG-LFCN block was performed in group PL. After confirming the efficacy of nerve block,total intravenous anesthesia was carried out to maintain bispectrality index values 40-60.Postoperative sufentanil and tropisetron were used for PCIA. The consumption of propofol ,opioid analgesics during operation,postoperative recovery, and the incidence of quadricep muscle weakness at 1h after surgery,the incidence of adverse reactions and the satisfaction score of postoperative recovery and the occurrence of adverse reactions within 48h after surgery was recorded. Result: There were no significant differences in intraoperative medication, postoperative recovery and postoperative adverse reactions between the two groups, but the incidence of postoperative quadriceps muscle weakness in the group PL was less than that in the F group at the same time (P<0.05), and the postoperative recovery satisfaction score of the group PL was significantly higher than that of the group F. Conclusion: Fewer drugs resulted in higher postoperative recovery satisfaction. PENG-LFCN block is worth promoting.

    Clinical Study of Compound Duzhong Jianggu Granules Combined with Glucosamine Sulfate in the Treatment of Knee Osteoarthritis

    Objective To investigate the clinical effects of a compound Du Zhong Jianggu granules combined with glucosamine sulfate capsules in the treatment of knee osteoarthritis. Methods A total of 104 patients with knee osteoarthritis who visited and were hospitalized in the hospital between January 2020 and March 2020 were selected and randomly divided into two groups of 52 patients: the control group patients received oral glucosamine sulfate capsules, 2 capsules / time, 3 times / D on a daily basis. Patients in the study group received compound Du Zhong Jianggu granules at 12 g / time for 3 times / D daily on the basis of oral glucosamine sulfate capsule therapy. After the two groups were consecutively treated for 6 weeks, the clinical outcomes of patients in the two groups were observed and compared, and the serological parameters including: Serum Hypersensitivity C-Reactive Protein (hs CRP), cyclooxygenase-2 (COX-2), cartilage oligomeric matrix protein(COMP) and matrix metalloproteinase-3 (MMP-3) were collected before and after treatment, respectively, and the above data were collected and compared. the above data were collected and subjected to comparative analysis. The visual analog scale (VAS) scores and Lysholm knee scores before and after treatment in the two groups were collected for comparative analysis. Results After 6 weeks of treatment, the therapeutic response rate of the control group was 80.77%, which was significantly lower than that of the study group (94.23%, P < 0.05). The levels of hs CRP, COX-2, comp, and MMP-3 were significantly lower in both groups, and the values of the same groups were significantly different before and after treatment (P < 0.05, P < 0.05), both of which were significantly lower than the control group (P < 0.05). After treatment, the VAS scores of both groups were significantly lower, but the Lysholm scores were significantly higher (P < 0.05). After completion of the 6-week treatment, the Lysholm scores of the study group were higher than those of the control group, and the VAS scores were significantly lower than those of the control group(P<0.05)。   Conclusion The compound Du Zhong Jianggu granules combined with glucosamine sulfate capsules can achieve good efficacy in the treatment of knee osteoarthritis, and this method has better clinical efficacy than glucosamine sulfate capsules alone.

    Clinical Observation on the Treatment of Infectious Bone Defects with Prunella Vulgaris Decoction

    Objective: To study the clinical effect of Prunella vulgaris decoction in the treatment of infectious bone defects, and analyze the application methods in clinical treatment. Methods: 100 patients with infectious bone defects admitted to our hospital from February 2020 to February 2021 were selected and averagely divided into an experimental group and a control group. The control group was treated with conventional drugs and methods for infectious bone defects, while the experimental group was given Prunella vulgaris decoction along with conventional drugs. The cure rate of inflammation, equal length rate of lower limbs, rate of knee joint stiffness and satisfaction rate of patients in both groups were recorded. Results: In the control group, the cure rate of inflammation was 88%, the equal length rate of lower limbs was 92%, the rate of knee joint stiffness was 32%, and the satisfaction rate of patients was 82%. The figures in the experimental group were 100%, 96%, 10%, and 98%, respectively. Conclusion: Prunella vulgaris decoction is effective in treating infectious bone defects, effectively relieving patients’ symptoms and helping them recover. Therefore, the application of Prunella vulgaris decoction in treating infectious bone defects should be strengthened to improve the therapeutic efficiency and promote the better recovery of patients.

    The Correlated of Serum Neutrophil Gelatinase Associated Lipid Transport Protein Levels and the Acute Coronary Syndrome

    Objectives: To study the serum neutrophil gelatinase associated lipid transport protein levels and coronary heart disease. Methods: 39 enrolled patients were divided into acute coronary syndrome (ACS) group (20 cases) and stable angina pectoris (control) group (19cases) according to the clinical manifestation and angiographic results. High-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), alanine aminotransferase (ALT), glutamic oxalacetic transaminase (AST), serum creatinine (Cr), Serum neutrophil gelatinase associated lipid transport protein (NGAL) were measured by Elisa test. And calculated the Genisi score of every patient. The association of NGAL with severity of coronary heart disease was studied. Results: that there were no significant differences in age, gender, BMI, smoking, hypertension, diabetes, ALT, TG, LDL-C and Cr between the control group and ACS group(P&gt;0.05). The difference of Gensini score, NGAL, AST were statistically significant(P&gt;0.01). The ordered Logistics regression analysis showed that the NGAL was an independent risk factor for coronary heart disease. Conclusion : NGAL may serve as an independent risk factor of CHD.
